Under data protection law you, as a patient of Graeme Cooper Chiropractic Ltd, have specific rights. This privacy notice explains why we collect information about you, how that information will be used, how we keep it safe and confidential and what your rights are in relation to this.

DATA PROTECTION OFFICER

As we record and use sensitive data we take the protection of this data very seriously. We have therefore appointed a Data Protection Officer, Louise Cooper, who is your first point of contact for any matters regarding your personal data we process. WHY WE COLLECT INFORMATION ABOUT YOU

Health care professionals who provide you with care are required by law to maintain records about your health and any treatment or care you have received both in the private and NHS healthcare system. These records help provide you with the best possible healthcare and help us to protect your safety.

We collect and hold data for the purpose of providing chiropractic services to our patients and running our clinic which includes monitoring the quality of care we provide. In carrying out this role we will collect information about you which helps us respond to your health and queries. We will keep your information principally in written form and/or digital form. The records will include basic personal data such as name, address, date of birth, telephone numbers and email address. They will also contain more sensitive information about your health and also information such as outcomes and treatment protocols.

 

DETAILS WE COLLECT ABOUT YOU

The health care professionals who provide you with care maintain records about your health and any treatment or care you have received previously(e.g. from Hospital specialists,GP surgeries, A&E, other healthcare professionals) These records help to provide you with the best possible healthcare.

Records which Graeme Cooper Chiropractic Ltd will hold about you will include the following:

 

          *Details about you, such as name ,address, DOB, contact information

*Any contact the clinic has had with you, such as appointments, clinic visits,  emergency appointments.

*Notes and reports about your health

*Details about your treatment and care

*Results of investigations, operations, procedures and tests/scans

*Relevant information from other health professionals

 

SHARING YOUR PERSONAL DATA

We only share your personal data with your explicit consent, where, for example you ask us to contact a third party, e.g. such as a solicitor, and give them your contact details and copies of health records and treatment details in order to make a personal injury claim following a road traffic accident.

Where third parties are used by us to store your personal data, we ensure they are compliant with the data protection law and any such data is not stored outside of the EU.

 

RETAINING YOUR PERSONAL DATA

Whilst you are an active patient we will continue to store and use your personal data. If you become an inactive patient we will retain your personal data for a period  at least 7 years following the last occasion on which the treatment was given. In the case of treatment to minors, records shall be kept for at least 7 years after they reach the age of majority (18).

 

YOUR RIGHTS

Individuals are provided with legal rights governing the use of their personal data. These grant individuals the right to understand what personal data relating to them is held, for what purpose, how it is collected and used, with whom it is shared, where it is located, to object to its processing, to have the data corrected if inaccurate, to take copies of the data and to place restrictions on its processing. Individuals can also request the erasure of their personal data.

These rights are known as Individual Rights under the Data Protection Act 2018.

WHAT IS A PRIVACY NOTICE

 

*        You may request a copy of your data at any time. Please make such a request in writing or by email to the Data Protection Officer (DPO), whose details are shown above. Please provide the following information: your name, address, telephone number, email address and details of the information you require. We may charge a reasonable fee for the administration of the request.

*        If you believe any of the personal data we hold on you is inaccurate or incomplete, please contact us directly and any necessary corrections will be made without undue delay so our records for you are accurate and up to date.

*        If you believe we should erase your data, please contact the DPO, whose details are shown above.

*        If you wish us to stop storing or using your data, please contact the DPO, whose details are shown above.

*        Where you have provided explicit consent for us to use your data, you have a right to withdraw this consent at any time.

 

You should understand when exercising your rights, a substantial public or vital interest may take precedence over any request you make. In addition, where these interests apply, we are required by law to grant access to this data for law enforcement, legal and/or health related matters.

 

If you provide us with your mobile telephone number we may use this to send you reminders about your appointments or other health information. Please let us know if you do not wish to receive reminders on your mobile telephone.

 

DATA BREACHES

Should your personal data that we control be lost, stolen or otherwise breached, where this constitutes a high risk to your rights and freedoms, we will contact you without delay. We will give you the contact details of the DPO who is dealing with the breach, explain to you the nature of the breach and the steps we are taking to deal with it.

 

COMPLAINTS

If you are dissatisfied with any aspect of the way in which we process your personal data please contact our DPO. You also have the right to complain to the UK’s data protection supervisory authority, teh Information Commissioner’s Office (ICO). The ICO may be contacted via its website which is  https://ico.org.uk/concerns/, by livechat or by calling their helpline on 0303 123 1113.
